Why does the first file always wait for my review?
The first file of a configuration, and the first after a change to it, the site or the columns, always waits for your confirmation, even with automatic publishing on.

Because nothing has proved yet that the configuration does what you mean. The first file published with a configuration version, a changed mapping, a change in the site's fields, or different columns than the last published file all ask you to tick I reviewed the changes above before publishing, and automatic publishing waits for you in those cases.
Once a person has approved a version, later files of the same shape can publish on their own when nothing needs attention. Changing only the notification setting or the configuration's name does not make the next file wait.
What needs your attention lists each reason with its message.
